Neck Pain

1. Call our office at 405-848-7246 to schedule your first appointment. Please bring all imaging records, list of previous treatments as well as a list of current medications.  We are more than willing to request those records for you ahead of your visit.  We also encourage you to bring your spouse or loved one to your initial visits.

2. Fill out the following paperwork in full and bring with you for a faster process

3. Your initial consultation is for Dr. Philipose to determine if you qualify to be a patient at our office. We may order additional imaging or labs if needed.

4. Once your case has been reviewed, Dr. Philipose will design a personalized treatment plan and review treatments, insurance/out-of-pocket, and schedule.  Once the treatment plan is agreed on, the case proceeds.
